Apparent Age: 20 yrs. Height: unknown, Girth: 37 ½”, Weight: 184#, Complexion: Fair, Eyes: Blue, Hair: Dark Brown. Scar ½” above right nipple, round scar. Medical: “Fit”. 31/08/14 at Valcartier, PQ. Last address:  Astard St., Detroit, Michigan, USA. Samuel was Disc, 9/11/15 – Ward 5 of London, ON. He served with the 7th Reg. (Fusiliers 1914 – 12/08/14 – To 22/09 to October 31 

30/01/15 – Hand Grenade in right forearm. Weakness in right Arm, unable to do heavy work with limb limb. 21/02/16 – at Medical Board: GSW Arm & head. “Recovered” – Fit for Duty 16/07/16. Last Pay ;- $617.63  

Reported that while serving with the 36th Bn. was wounded by shrapnel in the Right Arm and  neck. Admitted 30/10/15. Unit sailed 3/10/14. He would be transferred to England for at least six months for hospital.

25/04/15 Sharp Wd. Head, GSW Neck, Transfer 27/04/15 – 11/05/15, 13/05/15, and 16/05/15 rejoined. 1/06/15 Australian Hosp. Sharp Right Arm to England. 31/05/15 Admission – trans. from CAMC –GBS: 28/12/15 kept on “Permanent grey ‘Private’.  

5/05/15 Wounded and admitted to the Australian Hosp. at Wimereux for treatment of shrapnel wound to his arm – 10/05/15 – 6 Gen. – Rouen disc – 10/05/15 GSW-head wound to a convalescent camp.   24/05/15 – Shrapnel wound to his arm treated at the Australian Hosp. Wimereux. 31/05 Shrapnel wound to his right arm.  

15/06/15 at Corcelette, received several shell wounds of his right arm and fore arm and head – 20/07/15 Conv. Hospital – Bromley. GSW & Arm. 28/09/15 – Conv. Hospital – Epsom, Disc. to unit. 20/10/15 – Cen. Mil. Hospital – Shorncliffe 20/10/15 – Disc. 20/10/15 –Ex. Mil. Hosp. – Shorncliffe 

Returned to Canada 19/06/18 – 23/12/18 – Embarked for Canada. 4/02/19 – Discharged, London, ON.

Transferred to England per H.S. St. Davis – Bethnal Green War Hosp. at London. 8/06/15 – Shrapnel, Cdn. Conv. Hospital 20/07/15 – GSW Right Arm – Bromleg. Can. Com. – Epson 28/09/15 – GSW, Disc. 20/10/18 – GSW, Discharge – Military – Shorncliffe 20/10/15 – Discharged age 24 years.

18/02/17 Transferred to Hastings 18/02/17, 22/02/17 Hastings 14/12/18 “On Command” 1st Buxton. 

Returned to Canada 19/06/18 – 23/12/18 – Embarked for Canada. 4/02/19 – Discharged, London, ON.

It was noted that Mrs. Whitmore (mother) 312 Custer St., address on discharge: 385 Carrie Ave., Detroit, Michigan. 4/2/19 – Dish. aged Medically unfit. Discharge Cert. served with 1st Batt. – Medically Unfit. GSW – Wounds to  his neck – Multiple scars along with his Right Arm. Discharge 4/02/1918.
