Apparent Age: 40 yrs. 2 mths., Height: 5’ 81/2”, Girth: 38”, Complexion: Medium, Eyes: Blue, Hair: Brown. Tattoo: Left Arm.  Medical Exam: Considered “Fit”. At Chatham, ON. 1915 MEDICALLY UNFIT.  (Ship name unreadable) 18/04/1 

33rd Bn. 5th Contingent) 1904/15 to 15/06/15 ‘left under sealed orders’. Unit sailed 13/03/16. 400081 1st Batt. Cdn. Inf. – Theatre of War:- FRANCE – Date of Service 23/08/15 (Emily Street – Wallaceburg – Box 592.

CDP 15/01/15. Wounded at the front and hospitalized for eleven weeks Charles was considered unfit for further military service. Admitted to No. 4, General Hospital – Camiers, Apr. 1st GSW. Foreman: L- Shell Shocked with 1st Batt. (A334) 11/04/16. 341 Dept. – Etaples 18/04/16. GSW: – Right Forearm plus Shell Shock No.4 Gen. Etaples – No. 4 23/04/16 – Shell Shock. Moved to Moore Barr. Cdn. Hospital 9/08/16 = also with Psoriasis – Discharged 24/08/16 to Discharge. 

23/04/16  “Classified P.B. and transferred to 36th Bn. Res. Bn. – West Sandling.”

1916 – Paniasis:- He has had this disease at intervals for the past fourteen years. Right leg – below his knee. Disease: 1st time affecting his scalp. 28/08/16. Discharge – “Fit for Duty – but Discharged 24/08/16 

14/07/17 TOS. – Buxton – Embarked for CANADA from LIVERPOOL, 14/08/17 – Dis. Dpo. o/s Medical unfit at Quebec 23.09/17. Proceeding on Discharge: Age: 50 yrs. “Star” Charles had served in South Africa 1899 – 1902. Was in France 10 ½ months. Discharge: 6/09/1917.

The Dept. 0f Veteran’s Affairs: Warwick, Charles Henry – 400081 – WWI – DVA. Tele Memo, at Windsor, ON. 30 June, 1968. DATE OF DEATH: 29/01/1968 – Place of Death: Wallaceburg, ON.    He was back in Wallaceburg in September 1917 and would remain there later becoming the police chief.
