Son of Willmott Underhay (father) of Ridgetown, ON. Vaccinated. A single salesman living at 243 Queen’s Ave., London, ON. Previously served in His Majesty’s Forces,  Attestation: 12/10/1917 at London, ON. to no. 1 Training Depot.

Apparent Age: 20 yrs. 7 mths. Height: 5’ 6 ½”, Girth: 37”, Weight: 150#,  Complexion: Dark, Eyes: Brown, Hair: Dark, Eye sight: 20/20, Hearing R&L Normal. Medical “Fit” 12/10/17 at London, ON. Triangular – 2” scar on left buttocks – boil pre-enlistment C.A.M.C. (TD # W). 

Date of Discharge 5/05/1919 at Western Ontario Military Hospital. Had ‘Victory Bonds” – $51.82. “ moderate goiter on enlistment. On 6/04/18 a fine tremor developed which has since enlarged thyroid with symptoms. Fine tremor C11.
