Her next of Kin: Alfred Danby Smith, 427001, B-Coy. 46th Batt. CEF, (husband).  She also had a son in service. Attestation: RAMC (not known), CAMC 9/03/16 at London, England. Age 38 years, 9 Mth.

Pre-enlistment add: 75 West Side, Chapham, Connon S. W. London, England. Also Alfred Danby Smith “B” Co. 46th Bn. at Bramshott 23/10/16 1916.  

Attestation: 9/03/16, Age: 38 years, 6 mths. Height: 5’ 6”, Complexion: Fair, Eyes: Blue, Hair: Dark. One vac. Scar left arm, Weight: 115#.   Medical: “Fit”. 9/03/1916 at London, England. 

She enlisted in the RAMC at 75 West Side, Chapman Common, S.W. London, England 23/03/16. Posted to Moore Barracks Hospital 21/03/16. Transferred to Westcliffe General Hospital 21/10/16. She was reported on a 6 week sick  at Shorncliffe. She was suffering from Herpes Zoster (Shingles) in October 1916. 

Herpes zoster is infection that results when varicellazoster virus reactivates from its latent state in a posterior dorsal root ganglion. Symptoms usually begin with pain along the affected dermatome, followed within 2 to 3 days by a vesicular eruption that is usually diagnostic.

Transferred to A.D.M.S hospital Shorncliffe, London, 8/11/16, she was reported on sick leave in November. She was down with Gastroptosis [an abnormal downward displacement of the stomach causing digestive symptoms and constipation] in early February 1917.  Her last assignment with the RAMC was at Moore Barracks Hospital.  

Her transfer to the CAMC came effective 8/03/17 and was assigned to No.1 Cdn. General Hospital. She would spend three months in France at No.2 General Hospital – Estaples, Fr., from 27/05/17 to 14/08/17 as a ‘reinforcement’. She was returned to England to recover a bout of Vertigo 16/08/17. At that time her husband and son were both serving in France.   NS. Smith would return to England assigned to Q.A.I.M.S. Hospital in London arriving there 24/08/17.

After so much illness ( Herpes Zoster, Gastroptosis, Vertigo. She was reported for any service for 6 weeks. After two months she was fit for General service. She was reported for two months, then 3 weeks and finally “Permanently Unfit for any service.   A Medical Board ruled that she was discharged as “Permanently Disabled”, and she was permitted to resign her appointment with the CAMC. She did qualify and was granted a Gratuity of $50.00. A NS weekly wage was 108 -111 dollars. 

Proceedings of the Medical Board: – 12/04/17 at 76 Strand, A.D.M.S., London area. RE: NS. M L D SMITH. age 40 years, Service 13/12 Gastroptosis, present disability 12/04/17. “ The states that she is now eating and sleeping well and feels quite strong. She is still wearing an ‘abdominal belt”. The result had her “Fit for General Service.” The Board deemed her “Not fit for duty – 7/09/17 

30/10/17 “ The resignation of N/S. M.L.D. Smith has been accepted with effect 31/ 10/17.  “this officer has been in Westcliffe Hospital suffering with herpes Zoster since 18/10/1916 and she is now able to be out but has some tenderness and has failed degree of debility and has lost some weight.
