Apparent Age: 31 years 5 mths. Height: 5’ 5”, Girth; 37 ½”, Complexion: Medium, Eyes: Blue, Hair, Dark Brown, Weight: 155 #. One scar on left ankle Medical 4/07/16 at Hamilton, ON. “Fit”.

Served in Canada for July to August with the Cdn. Mtd. Rifles. 

TOS. 28/08/16 for embarking at Halifax, NS. to depart for England 15/08/16.  Philip arrived in England aboard the S.S.Grampian 24/08/16. 6/10/16 transferred to 11 Res. Bn. 

14/11/16 was reported to No. 2 General Hospital – Infuenza.  24/11/16 Taken to #2 General Hospital  – Havre 14/11/16. Transferred to #4 Conv. Hospital – Havre 18/11/16 to 24/11/16 when he was discharged from hospital. Pte. Philip Henry Rodgers died of his wounds at No. 11 Gen. Hospital – Camies – DOW.

Mrs. Fuby Rogers the mother of Philip was deceased that would leave the Memorial Cross to his sister Miss Ethel Wallace Crookston, ON. Walter Rogers of Mador, ON. Received the Memorial Scroll 1/05/21 and the Memorial Plaque 9/10/21
