Apparent Age:- 31 yrs. Height: 5’ 6 ½”, Girth: – 36”, Complexion: Fair, Eyes: Blue, Hair: Light Brown. Tattoo VR on left forearm. Vaccinated – Yes.  Medical “Fit” 14/01/16 at Woodstock, ON. to 186th Bn. CEF:- 1901/16. No Military training. Attestation: 14/01/16.  Present address: Blenheim, ON.

Pte. Owen was was listed on the “Particulars of Family of an Officer or Man Enlisted”:- ie: Service No. 675817 s/b 675187, ‘Owen’ should be Percy not Charles.  

1st Bn. France, 22/04/17. Embarked from Halifax sailed aboard the S.S. Lapland 30/10/16 as 168 Batt., 14/01/16 to Nov. unit sailed 30/10/16. “Theatre of Service – UK, personnel “ Disembarked at Liverpool – Transfer to 12th Batt. – West Sandling 5/12/16, TOS. 12th Bn. West Sandling . TOS 4TH RES. 4/01/17.

Arv. in FRANCE 22/04/17. ARV. – FIELD – 30/04/17 . Percy was granted a 14 days leave to England – 28/01/18 – returned 13/02/18. Proceeded to England 25/03/19 – Proceeded to Canada.  

Date of Discharge – 24/04/1919. (noted 12/04/21 – Owen P. – 1st Bn. “Chronic Otitis Media Supperativa – had from Childhood. Place of origin, England. “No infectious disease.” Deafness and discharge – right ear.” Deafness and discharge right ear.” Dated from childhood – “Discharge from right ear ‘aptoa few days ago.” Left ear discharged for a long time.” Before enlistment serious – Persistent. Can return to occupation.      

After his return he attended the Institute of Public Health – Capt. Dr. Keillor. Rec’d 9/07/19 during his Physical Exam. Fluids in his ears – App. Cloudy, with sediment – slight , Odor: Fresh, Color: Normal – not noted of reason? It was noted that Percy had ear problems from enlistment to discharge. It was noted 24/01/17 at Westcliffe. Percy had Def. Hearing: “Have examined the marginally named man and find him suffering from chrome patarrhal Improves to 22 and 25 feet on treatment. Fit for category A1, returned here only a week while in England for treatment. J. A. Stewart – Captain – CAMC. for o/c Westcliffe Hospital. – On demobilization medically – general service.
