BOARD at London, ON,. 4/10/17 – Chronic Suppuratibe Otitis Madia – Examined by S.S. Board at London, ON. – Has been in hospital since 14/08/17 – With Otitis Media. No discharge at present. Hearing Impaired, can hear ordinary voices at 15 feet, no disease, perforated ear drums, Specialist (report attached) Heart and other organs – Normal. “Tip pf ring finger – missing right hand. Linear – Scar ½ “ long on inner side of terminal phalanx of 2nd finger.”
