Attestation: 9/02/1918 at Winnipeg, Manitoba to CFA. He had no military experience.  Medical Exam at Winnipeg, Manitoba 9/02/18. Declaration of Recruit:-

Apparent age: 21 yrs. 3 mths. Height: 5’ 9 ½ “, Girth: 35”, Complexion: Fair, Eyes: Blue. Hair: Dark Brown. Eyes 20/40 R & L. Hearing – Normal. At Winnipeg MB. 9/02/18. Medical History – never vaccination – had one tooth for repair 1Y. 18/04/19. A mole on neck. He did have Scarlet fever.
