30/04/19 – Discharged in consequence of “On Demob:- Ilization in accordance with R.O. Nos. 1328 & 1420 – 1-D-97- C – 196-D/28-4 – 19 D.O. No. (120 para at part (2) D/30/04/19 with effect. 30/04/19 ‘Certified correct’  1st Bn. Cdn. Garr- Regt. 

Medical History ;- last birthday 23yrs. – 29/07/1895 – Date of Birth. Attended in American service in Russia 10/21/18 – 25/08/18 to 25/08/1918 to 27/10/19 – Russia 27/08/18 – took a Bullet while in service with the American Army.

 The board found anterior dircumation limited by 15 Posterior cireumduction – normal abduction and adduction normal. All other movements of the arm – normal. This disability had its’s original while a desertion from the Canadian Army and with the American Army – there fore no disability was due to Canadian servitor. “Fit for service aboard not general service. 29/04/1919 at London, ON.

While in Action with American forces in Archangle – Russia 27/08/18 – received a GSW in the left shoulder taken to No. 29 Base Hospital – Archangel wound did not sufferate – recovered in 2 months – Invalided back to the USA – 22/12/18.
