His next of kin: Mrs. John Carrick. Wellington was a physician living at 144 Grande Allee, Quebec City, PQ. Member of #12 Field Ambulance CAMC. Attestation: 14/09/1918 at Quebec City. To C.A.M.C.
He attended the University of Toronto Faculty of Medicine and graduated in 1905. He then spent 18 months taking post graduate training at a local general hospital in New York for almost three years. Dr. Carrack entered private practice in 1914.
Captain Carrick served in the 36th Battalion in 1915 his unit sailed for England 19/06/1915. He transferred to the No. 5 Division.
With the outbreak of the Great War, he offered his services in 1914. He was the deputy assistant director of the medical services for the Military District No. 5 and the assistant director of medical services at Valcatier, PQ.
He rose to the rank of Major in the 10th Field Ambulance in Siberia in 1918.
Major Carrick embarked 26/12/18 aboard the S. S. Emoress of Russia and arrived in Siberia 15/01/19. He returned to Canada 19/05/19 and was TOS 6/06/1919 at Hastings Park in Vancouver, BC. Major Carrick was struck off due to demobilization at Toronto, ON. 20/06/1919.
After the CSEF returned to Canada, he resumed his practice and was in the x-ray department of the Mayo Clinic in Rochester, Minnesota for two and a half years. He later joined the staff of the Mowbray-McGregor Clinic in Hamilton, ON, and resumed his practice as a specialist in radiology.
