“This man gives a history of two Severe Empynembia – for 10yrs., ago from which he did not recover for three years. Breathing in lower and middle lobes of the right lung is very faint and almost in perceptible. There is a dullness on percussion over these lobes, Expansion Re: Leasened. Vocal fremitus is decreased over the whole lung. Circulation is poor, extremities are blue and cold even the cars. Feels as well now as when he enlisted.   

Carded 6/06/17 – Scar 4” long in line with the 8th rib. Result of an operation right side. Scar 1” long on level with the right Nipple and 2 ½” posterior.

Discharge:  Further care and hospital treatment will NOT improve this mans condition and it is advisable that he pass under his own control. (16/05/17).
