Medical Report: Double Uvula, Split Palate – Is able to swallow, Difficulty of speech. Bunions on both of Great toes. Suffered from Inpetago 29/12/16 to 15/01/17 recovered at Military hospital – London, ON. 29/12/16. Ringworm 15/01/17 treated 

Date of Discharge: 9/03/17 at London, ON. “Medically Unfit”, causes “not due to military service.”
