Son of Walter Ballard of 5 Beauclare Buildings, Norman St., Chelsea, London S.W., England. It was presumed other family members were in England.

A single farmer living at R. R. # 2 Thamesville, ON. CEF. Family, the father and presumed other family members are presumed in England. Complexion: Dark, Hair: light brown, Eyes: grey, Height:5’ 4”, Girth: 33 ¼ “,  Weight: 130#, Age: 19 Years. (Actual age: 16 years) 

Attestation: 26/02/1916 at Florence, ON. to the 149th Bn. It was reported on active service from 7 July to 29/07/1916. It was determined he was much younger than he led on. It was discovered that he was only 16 years old August, 1816, making him a few months short of being 18 years old. 

A Medical Board  held at London, ON. 29/06/16. It was determined he was under age 

He was marked as being medically unfit for service in London, ON., in fact the real reason was he was under age and  Medically unfit.
