Living at 210 Forest St., Chatham, ON. with his wife Addie. A farmer.  Attestation: 30/03/1916 at Chatham, ON. to 186th Bn., For 186TH BN. O/S.

Apparent Age: 41 yrs. Height: 5’ 3”, Girth: 35”, Complexion: Dark, Eyes: Light Brown, Hair: Black. Medical 30/03/16 at Chatham, ON.  

Children: Mildred 11 yrs. and Levi 9 yrs. His father alive: Robert Gallippo of 210 Forest St., Chatham. His mother Mary  – as above. He had applied for ‘Separation’ Allowance. He was not insured. 

Date of Enlistment 30/03/17. He was at Camp Borden 13/09/16 and had a Routine exam. 

Medical Board: 3/04/17. Levi had come down with Bronchitis 14/09/16 to 19/09/16 at camp with 186th Bn. 30 Apr. to 22/03/17 Renal Calculous – Unknown was kicked by a horse and was followed by pain in the back.

“Patient has a severe attack of pain, occasionally in right Lumbar region radiating to the left side and down the right thigh which lasts for three or four days. He had one attack since coming to the hospital 4 March. 1917.

Urine – Acid, Sp.gr. 102, trace of albumin, Pus and blood cells, x-ray shoes stone in right Kidney. Heart and Lungs normal, otherwise fit.”    

Scar on instep of right foot from injury before enlistment 1910.

Treatment would have to be done at the London Military Hospital for a Permanent unless removed by operation 

DISCHARGE – Refuses treatment and has signed wavers, He should pass under his own control 3/04/1917. “No Disability due to military Service”.
